STEP 1. Download coffee order form and give it to your Brigade/Missions Team Leader to record each members coffee order.
You have a choice of two formats from which to download your form:
- PDF format, if you want to fax the order to us
- MS Word, if you want to send it to us as an attachment to an email
STEP 2. Then, send us the order form via one of the following
methods:
FAX
- Fax to 425-790-4088
- For MS Word document - Email as an attachment to order@coffeeonline.org

- Record your order by providing the information as requested at www.coffeeonline.org/MDC_Order_Form.htm
STEP
3. Finally,
pay the invoice you will receive
from our accounting office.
Once we receive your coffee order, you will be given two options:
Option 1: Pay via credit card through Paypal and receive a $10.00 discount off your total order. This discount is only offered when you pay through Paypal upon receipt of invoice 10 days prior to delivery.
Option 2: Pay cash on delivery. Sorry, we do not accept checks; cash only. You will be receipted upon payment of your order. There will be a small administration fee of $5.00 for C.O.D.s. Payment must be in USA Dollars.
You will receive confirmation of your coffee order delivery information via the email address you provide on the coffee order form.
